Scholarship Acceptance Form

  • We are in the process of informing scholarship applicants of the status fo their application.  If you are been notified that you have been selected for a Sigma Alpha Mu Foundation scholarship you must sign and return a copy of the Scholarship Acceptance Agreement (download below) with a copy of your thank you note and a photo.

    Please be sure to mention the name of the scholarship in your note and address the note to 'Dear Scholarship Donor' as several scholarships are created by groups donors or in memory of Fratres.

    Your photo need not be formal (please no hats or t-shirts) and groups shots of scholarship recipients from chapters would be acceptable.

    If you have been selected for more than one scholarship please write a thank you note for each.
